RCIA QUESTIONNAIRE
The Diocese of Bridgeport has developed a standard questionnaire to be filled out by all catechumens and candidates at the beginning of the RCIA process. Its purpose is to provide all the information necessary for proper reception into the Church and handle any questions regarding marriage status in a timely manner (See Bridgeport Pastoral Guidelines for Implementing the Rite of Christian Initiation of Adults, no. 53). The information provided by participants in this form is confidential and should be handled as such. The questionnaire and its information are for parish use. These are not to be sent to the diocese.
