ADULT CONFIRMATION—INFORMATION SHEET

All fields are required fields. If the information does not pertain, put “Not Applicable” in the space.

When you have completed this information sheet, follow these steps:
1. hit “submit” and the information will be sent to the Director of Adult Formation.
2. Open the confirmation email you will receive and hit “print” and ask your pastor to sign the bottom of the information sheet.
3. mail the information sheet with the pastor’s signature AND a COPY OF YOUR BAPTISMAL CERTIFICATE to Gina Donnarummo, Office for Pastoral Services, 238 Jewett Avenue, Bridgeport, CT 06606. (If you are taking the Diocesan classes at St. Luke Parish from April 10 to May 22, the registration fee is $50.)
4. As you are choosing your sponsor for Confirmation, please keep in mind the person must be 16 years of age or older, have received the Sacraments of Initiation (baptism, confirmation, and Eucharist), and be an active, practicing Catholic. The person cannot be your mother or father. The sponsor should ask his/her parish for the sponsor certificate and the candidate should hand in the sponsor certificate to Gina Donnarummo.
